Pretty Princess, LLC Participation Release and Waiver
This agreement is between
PrettyPrincess,LLC D.B.A. PrettyPrincessBeautyBus(hereinafter“Company”)and the below signed customer (here in after“Participant”),or the legal guardian,(here in after“Guardian”).This agreement governs the terms of participation in activities including,but not limited to,mini manicures and pedicures,facial sheets,make up application,hair dressing, and paint and sip parties.The activities shall be collectively referred to as“Activities”and their associated risks shall be further described below.This document shall be effective as of the date it is signed by both parties.
Assumption of the Risk
Participant in tends to voluntarily participate in child friendly days pa activities provided by Company.While we strive to minimize risks too ur customers there is apossibility of exposure to potential skin and lung irritants and allergens.Risks include,not by way of limitation,exposure to various paint mediums,aerosolsused in nail polish and hair products,and other potential allergens and contaminants contained in products like paint supplies,facial sheets,and makeup.
By signing this agreement and engaging in the Activities,Participant and Guardian here by assumes the risks in herentinsuchparticipationandsignifiestheyarenotphysicallycompromisedorlimitedinanywaythatwouldimpacttheirabilitytoparticipateintheActivitiesthatCompanyprovides.Companyisnotresponsibleforanyinjury/damagethatmayoccur.
ReleaseofLikeness
ParticipantunderstandsandacknowledgesthatCompany,orothers,maytakeandusephotographs,film,audiorecordings,orimagesoftheirparticipationinActivitiesprovidedbyCompany.Participantherebywaives,inperpetuity,anyrightofprivacy,publicity,compensation,intellectualproperty,oranyotherrightstoanyimages,videos,orrecordingsandfurtherconsentsthatCompanymayuseoreditsuchimages,videos,orrecordingsinanywayforanypurpose.
OtherProvisions,Rules,andProtocol
CommunicableDisease:Participantacknowledgesthecontagiousnatureofcommunicablediseases,includingCoronavirus/COVID-19.CompanycannotguaranteethatParticipant,oranyotherparty,will not become infected withCOVID-19oranyothercommunicabledisease.ParticipationinActivitiesprovidedbyCompanymayincreaseParticipant’slikelihoodofexposureandinfection.BysigningthisagreementParticipant/GuardiansignifiesParticipantisnotexperiencinganysymptomofillness,thattheydonotbelievetheyhavebeenexposedtosomeonewithasuspectedand/orconfirmedcaseofanyillness,includingCOVID-19,andtheyarenotawaitingtestresultstodeterminewhethertheyareacarrierofanycommunicabledisease.
PersonalProperty:Participantisresponsibleforthesafetyandsecurityoftheirpersonalproperty.Companyisnotresponsibleforanydamage,loss,theft,orothermisfortunethatmayoccasionParticipant’sproperty.
In consideration for participation in Pretty Princess, LLC. activities, and for other good and valuable consideration, Participant hereby confirms their understanding of the above terms and further agrees to waive their rights to sue Company for any cause including, personal injury caused by negligence of Company, its owners, employees, or agents.
